I am eligible to receive intervention from a therapist from TCCI. I understand that the practitioners will assist me in reaching goals which will contribute to resolving my presenting difficulties.

Should I be dissatisfied with the progress in achieving my goals, or if I am unhappy with the therapist, I will inform TCCI immediately.

Further, I understand that all information shared with TCCI is confidential and no information will be released without my consent. I further understand that there are specific and limited exceptions to the confidentiality outlined above, which include:

• When there is imminent danger to myself or to another person. TCCI practitioners are ethically bound to take necessary steps to prevent such danger.
• When there is suspicion that a child or elder is being sexually, physically, or emotionally abused. TCCI practitioners are required to take steps to protect children and elders and will also inform the appropriate authorities.
• In the event of another risk, I provide consent to TCCI to contact my emergency contact and to disclose such risk to them.
